Our facility is located in Suite 240 at 250 26^th Street in Prairie du Sac.  The clinic is located within Sauk Prairie Health Care and can be accessed through the main entrance.  The clinic is located on the second floor.

COVID-19 VISITOR GUIDELINES

As SSM Health considers the best way to care for our patients’ physical and emotional health, we recognize that family and loved ones are an important part of the care team. Effective Monday, March 8, 2021 we will allow one support person over age 16 for adult patients, and two support persons over age 16 for pediatric patients (0-18) at this location.

• Patients will be encouraged to bring a support person only when needed.
• Visitors must pass established COVID-19 screening protocol.
• Visitors must wear a face mask at all times with in the facility, and it must cover nose and mouth.
• Social distancing encouraged when in our facilities.
