Statistics show that the more a person is involved in his or her own care, the fewer complications that will occur. Our nationally-recognized and certified diabetes self-management education program (through the American Diabetes Association) can help you with the daily management of diabetes.

Our certified diabetes educators (certified with the National Certification Board of Diabetes Educators) work with your healthcare provider to develop your individualized diabetes management plan at sites located in Fond du Lac, Ripon and Waupun. Our staff includes both registered nurses and registered dietitians to meet your specific individual needs.
