SSM Health at Home staffs and operates the SSM Health Hospice House, a four-bed hospice residential facility in Baraboo for patients who, for whatever reason, are unable to remain at home during their illness.

While at the Hospice House, patients receive all the in-home hospice services, plus: meals, medication administration, laundry, and housekeeping. The Hospice House is staffed 24 hours a day.

Amenities

The Hospice House includes four spacious, private resident/family rooms, each with a bathroom and kitchenette. Common areas include a full kitchen, dining and activities room, lounge, meditation room, patio and garden/courtyard.

Residents and family members are welcome to take full advantage of all the Hospice House has to offer. Families may gather in the common areas or be together in the resident’s room.

The Hospice House provides a restful, serene environment for residents and families. Caregivers can spend as much time with the resident as they wish and take breaks when needed. Each resident room has a sleeper sofa.

Frequently Asked Questions

What should I bring for my loved one?

Please bring personal items such as a toothbrush, toothpaste, shampoo, and other grooming supplies. Comfortable, loose-fitting clothing such as sweat suits or pajamas are recommended. The house provides hospital garments if needed. It is also important for patients to have comfortable, non-skid footwear. We want your loved one to feel at home so personal items like photos, pictures, collectibles, craft, and hobby supplies are always welcome.

Are meals provided?

Morning and evening meals are prepared for patients by SSM Health at Home staff. The noon meal is ordered from the nursing home. Meals are not provided for the family, but feel free to use our kitchen to prepare meals. Patient rooms come equipped with small refrigerators, so you are welcome to bring in food for yourself or your loved one. Most patients eat very little and may prefer particular foods. Let our staff know your loved one’s preferences so that we can add these items to our shopping list.

When can I visit?

You are welcome to visit at any time. If you are worried about your loved one, you may call and talk with the aide staff 24 hours a day. For patient and staff safety, the door may be locked. Ring the bell and the attendant will open the door when they are available, as they may be providing patient care.

May I stay overnight?

Yes. There is a sofa sleeper and bedding available for those who wish to stay. However, we suggest that during short-term stays, family members take advantage of the opportunity to re-energize and take care of themselves.

Is there a registered nurse on-site 24 hours a day?

No, however, there is a registered nurse on call 24 hours a day, seven days a week. A Registered Nurse also makes visits to the house to see patients as needed.

How are patients’ medications provided?

All patient medications are required to be blister packed by a local pharmacy. We request family members coordinate with a hospice team member to facilitate this process.

What is the cost of staying at the SSM Health Hospice House?

The Medicare/Medicaid Hospice Benefit pays for hospice services such as visits by the hospice team members, equipment and medications related to the patient’s primary hospice diagnosis. These services are also covered by most private insurances. However, the daily cost of room and board is not covered by Medicare. If your loved one has private insurance you will need to verify with the insurance company to determine coverage.

What if I or my loved one is having financial difficulties paying for room and board costs at the SSM Health Hospice House?

If there are any financial concerns, please contact your SSM Health at Home social worker. There is financial assistance available through the SSM Health at Home Foundation of Wisconsin for those who qualify.
