SSM Health provides Department of Transportation or DOT physicals at locations through the region. Services include:

• DOT-certified physicals
• DOT drug screen and collections
• DOT breath alcohol testing (BAT)
• DOT medical review officer services

Preparing For Your Appointment

Drivers in need of a DOT-certified test or physical should bring the following items to ensure successful completion of the exam:

• Drivers license
• List of medications
• Bi-PAP/CPAP report, if applicable
• Physician’s written release to drive a commercial vehicle if you have had a surgery, major illness or injury within the last five years
