Telehealth Monitoring
SSM Health at Home’s telehealth program helps patients with certain chronic conditions remain in their homes during recovery, rather than face lengthy stays in, or readmission to, the hospital.
How Does Our Telehealth Service Work?
It all starts with a treatment plan created by your physician and your SSM Health at Home nurse. To support that plan, telehealth patients are provided with a monitor which prompts them to collect vital signs at predetermined times every day. The information is then securely transmitted to SSM Health at Home clinicians. If there is a concern, our staff contacts the patient. Depending on the situation, our staff will communicate with the patient’s physician regarding additional nursing visits, medication adjustments, or the need to visit their doctor.
SSM Health at Home installs the telemonitor in your home for 30 to 60 days at no cost to you or your insurance provider. An SSM Health at Home technician brings the equipment to you and will teach you how to use it.
Not only are you able to monitor, recognize and manage your own symptoms using the telemonitors, you can help decrease your chances of being readmitted to the hospital after using our service. In some locations, we also offer telehealth for monitoring chronic obstructive pulmonary disease (COPD), acute myocardial infarction and pneumonia.
